Check our insurance list.

If your insurance is accepted, fill out this form.
2. We respond!

We will let you know if we have a therapist that is a good fit and has availability.

If we do, we will schedule your first appointment and then get a file set up and send you required paperwork to be completed electronically through Therapy Notes.

If we don't, we will refer you out or add to our waitlist, whichever you prefer.
3. Paperwork

You will receive a welcome email from TherapyNotes prompting you set up an account through the portal.

Once your account is created, you will see the required paperwork in your portal.

Complete the paperwork within your portal which includes providing your insurance information, a payment method, answering some additional questions, and reviewing Consent for Services and Privacy Practices - which you can also view below if desired.
