Getting Started with RISE
We know the process of finding an accessible health professional can be complicated, confusing, and even overwhelming.

Although we aren't able to change how some of that process works, we have tried to make it as straightforward as possible by detailing the steps to work with RISE below. If you have questions at any point, please reach out and we'll be happy to help!
1. You Research & Request

Check your coverage eligibility with our insurance list.

If we are out-of-network (OON), you can pay our private pay rates and we can provide you with a superbill. Check your insurance to see what OON benefits you have first.​

If you are OON and don't want to pay private rates, consider seeing one of our interns for $25 per session.

If your insurance is accepted, or you're okay seeing an OON provider or intern, fill out our

2. RISE Responds

Once we have received your New Client Information Form, we'll determine if we have a provider that is a good fit and has availability that matches yours.

If we do, we we'll set up your file, send you required paperwork and questionnaires to be completed electronically through our EHR (TherapyNotes), and email you to confirm we've received your form.

If we don't have a provider that is a good fit, we will refer you out or add you to our waitlist, whichever you prefer.
3. Required Documents

After your intent is confirmed, you will receive a welcome email from TherapyNotes prompting you to set up an account through their portal.

Your required paperwork will be visible in the portal once your account is created.

Complete the paperwork within your portal, which includes:

Providing your insurance information

Adding a payment method

Reviewing the Consent for Services and Privacy Practices policies, also available below
